Managing your Health Online

Using an online form

There are many things you can do online at our practice. Some tasks you can do using our website where you can:

Contact us about your request

You do not need an account to use this website.

Using your NHS account or Systmone online

Here are other tasks you can do online, like:

  • order repeat prescriptions
  • view your test results
  • access your health record
  • check or cancel appointments
  • see your vaccination record

For these, you can use your NHS account (through the NHS website or NHS App) or Systmone online.

Setting up an NHS Account - NHS App

Owned and run by the NHS, the NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet.

The NHS App enables people to:

  • Book and manage appointments
  • Check their symptoms using the Health A-Z
  • Find out what to do when they need help urgently using NHS 111 online
  • View information regarding organ donation
  • Choose whether the NHS uses their data for research and planning

Patients can register and prove who they are so they can:

  • Order their repeat prescriptions and view, set or change their nominated pharmacy, where they want their prescriptions to be sent
  • View their GP medical record securely

All GP practices in England are connected to the NHS App. This means that anyone aged 13 and over and registered with a GP practice in England can use it.

You can find out more about NHS accounts and sign up on the NHS website.

Consult Online

Patients can now consult online with a clinician at our practice as part of a new online service which could save you time and a journey to the practice.

Using the online consultation service makes it easier for us to quickly decide how we can best help you with the least amount of disruption to your day. The service is free and confidential and medical consultations are always reviewed by a GP or nurse.

Simply answer questions about your symptoms online and we’ll get back to you with general advice or signposting, a telephone consultation or, if necessary, we’ll get you booked in for an appointment. You can use the system 24 hours a day, 365 days of the year.

Please DO NOT use this system if your query is medically urgent.

The service also offers access to reliable and trustworthy self-help generic information 24 hours a day, 7 days a week that may help you to avoid an appointment altogether.

A clinician will aim to resond within 2 working days, however if you make a specific request for a clinician this may take longer to receive a response.

Accessing your health record

Your health record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.

You can access your health record and nominate someone you trust to access it too.

You can use your NHS account (through the NHS website or NHS App) to access your health record.

You’ll need to ask the practice for online access to your full health record, or you’ll only see your health record from April 2022.

Request access to your full health record online

First, you need to register for online services and prove who you are. To request access:

  • Complete the Subject Access Request form (see below) and send in with photo ID so we can confirm your identity
  • If you are setting up access for the first time, we will give you a username and password
Access to other medical records

For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.

Requesting a copy of your records

If you would like to request a copy of you electronic record, please complete the Subject Access Request form and email it across to the practice at leyburnsurgery@nhs.et or hand in via Reception.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. Please discuss your requirements with reception and they'll provide the appropriate consent forms.
