Leyburn Medical Practice is an NHS general practice located in the market town of Leyburn, North Yorkshire. We provide primary medical services to our registered patient population and surrounding rural communities, delivering safe, effective and patient‑centred care.
The practice offers the full range of services expected of a modern GP practice, including general medical consultations, preventative care, health promotion and the ongoing management of long‑term conditions. We provide regular monitoring and support for those living with conditions such as diabetes, cardiovascular disease and hypertension. Family planning services and a range of additional clinics are also available to meet local health needs.
Our multidisciplinary clinical team includes GPs, Advanced Nurse Practitioners and Nurse Prescribers, all of whom are able to assess, diagnose and manage a wide range of conditions. They can issue prescriptions and make referrals to secondary care services where appropriate. The clinical team is supported by a highly skilled nursing team who deliver a broad programme of nurse‑led services, including chronic disease reviews, immunisations, health checks and preventative care.
We are committed to delivering care that is safe and effective. The practice follows national and local clinical guidelines, regularly reviews clinical performance and participates in audits to support continuous improvement. Robust systems are in place for safeguarding children and vulnerable adults, infection prevention and control, medicines management and the reporting and learning of significant events.
Leyburn Medical Practice aims to provide care that is caring and respectful, treating patients with dignity and compassion. We work hard to ensure patients are listened to, involved in decisions about their care and supported to manage their health as independently as possible.
We strive to be responsive to the needs of our population, offering a range of appointment types and making reasonable adjustments to improve access for patients with additional needs. We actively encourage feedback from patients and use this to shape and improve our services.
The practice is well‑led, with clear governance structures and a supportive team culture. Staff are appropriately trained, supported and supported to develop within their roles. We work closely with local NHS services and partner organisations to ensure joined‑up care for our patients.
